Quantitative acid-base dynamics in chronic pulmonary disease. Defense of pH during acute respiratory acidosis superimposed upon chronic hypercapnia.

چکیده
The defense of the extracellular pH was evaluated in 17 patients with chronic obstructive lung disease and chronic hypercapnia during superimposed acute respiratory acidosis. In group I the chronic stable state PaC02pH-H01was not defined; in groups IA and II the stable state was defined. Duration of acute acidosis in groups I and IA was 16 and 23i hr, respectively ; in group 11, 30 rain . The defense of pH in groups I and IA was greater than acute hypercapnia in normal man but less than chronic stable hypercapnia (P < 0.05), The pH in group II approached that of normal man during acute respiratory acidosis . Plasma bicarbonate concentrations paralleled the (H') responses. A wide confidence band based on these observations overlapped the previously established bands for acute and chronic hypercapnia . In the clinical situation, variable Paco, exposures create variable in vivo carbon dioxide titration curves. Thus, the defense of the extracellular pH during acute respiratory acidosis superimposed upon chronic hypercapnia is WAR temporally to renal buffering mechanisms. Under these circumstances, prediction bands for steady state acute or stable chronic hypercapnia are not valid for the interpretation of coexisting metabolic disorders .

Decrease in spectral entropy by low tidal volume ventilation-associated severe hypercapnia: a case report
Severe hypercapnia can be predicted by a decrease in cerebral electrical activity. The authors describe a sudden decrease in spectral entropy due to severe hypercapnia-induced respiratory acidosis in a patient with chronic pulmonary obstructive disease during lung resection. After two and a half hours of low tidal volume ventilation in the lateral position, the state entropy suddenly dropped fr...
متن کاملElectrolyte excretion in respiratory failure.
Patients with chronic emphysema and pulmonary insufficiency commonly arrive at a new steady state in which the carbon dioxide pressure in the blood is increased and the pH lowered; this situation can be prolonged for months during which these patients must be in acid-base balance. This equilibrated state of mild respiratory acidosis is, at times, upset by complicating pneumonitis which aggravat...
متن کاملRenal function in respiratory failure.
Respiratory failure produces significant changes in many aspects of renal function. Whereas in response to acute hypercapnia, body buffering mecha nisms are of primary importance in maintaining pH, in adaptation to chronic hypercapnia, increased renal excretion of acid is predominant. Augmented secretion of ammonia accounts solely for the net increase in renal acid excre tion. During chronic ...
متن کاملSkeletal muscle energetics, acid-base equilibrium and lactate metabolism in patients with severe hypercapnia and hypoxemia.
Quadriceps femoris muscle needle biopsies were performed in ten patients with chronic obstructive pulmonary disease and acute respiratory failure and in ten age- and sex-matched healthy control subjects. The main indices of skeletal muscle cell energy metabolism, intracellular acid-base equilibrium and lactate metabolism were evaluated. Reduced ATP and phosphocreatine content, intracellular aci...
متن کاملAcid-base balance, serum electrolytes and need for non-invasive ventilation in patients with hypercapnic acute exacerbation of chronic obstructive pulmonary disease admitted to an internal medicine ward.
BACKGROUND Hypoventilation produces or worsens respiratory acidosis in patients with hypercapnia due to acute exacerbations of chronic obstructive pulmonary disease (AECOPD). In these patients acid-base and hydroelectrolite balance are closely related. Aim of the present study was to evaluate acid-base and hydroelectrolite alterations in these subjects and the effect of non-invasive ventilation...
